Dermaroller (Microneedling) Realistic Expectations

Microneedling, is it worth it?

Treating acne scars is like painting a rainbow, to be realistic, it is never possible to use just one color.

Microneedling is just the indigo in a rainbow.  An effective acne scar treatment will be needing different treatments, different approaches and these can only improve a scar, erasing a scar is a false promise.

Acne scars are  common dermatologic complaints. One of the accessible treatment or instrument nowadays is a dermaroller, however, doing it yourself may not be a practical thing to do. Home use is never comparable to Clinical Setup performed by dermatologists especially when it comes to results.

We will be sharing with you our experience on the effectivity of Clinical Microneedling using the Skin wellness Protocol.

Let us give you a simple run through of the different types of acne scars and the effect of the dermaroller on each type.

 

Rolling scars or soft contoured scars are those that you can see right away, but you cannot point out where it exactly starts and end. This type of scar responds best to microneedling, it can sustain more stabs, more inflammation, more repair, and more collagen.

Box scars,  are quite sharp and well defined. This definitely needs some subcision, to release the retracting fibers on the base of the scar.

Ice pick scars are the very small scars that may seem to be just open pores, especially at the sides of the nose, but they area actually ice pick scars. This definitely needs a better treatment, like TCA cross, or a fractional laser.

Atrophic scars, are thin surfaced scars, just like cigarette paper. These can also be depressed scars. Microneedling with the help of some infusions of the epidermal growth factor can help improve the surface and add volume for a more even surface.

The other types of acne scars, the red hypervascular scars and hypertrophic scars are nice to mention, but they need a different set of treatment.

The severity is the depth of the acne scar, and will be described as mild, moderate and severe.

We have seen astonishing results with some of our patients, 80 to 90 percent subjective improvement in just 4 sessions. On the contrary, two patients, out of more more a hundred, seemed resilient to the wounding effect of the microneedling, less penetration, less bleeding, less repair process, thus getting poor effects, and shifted to different treatment options after 4 sessions.

Some patients with  severe scarring, despite doctor's advise, are so hopeful on single session of microneedling combined with subcision, some with very deep mixed scars expect dramatic results after 3 sessions of basic microneedling and ended up heart broken.

mild to moderate scarring

For mild to moderate acne scars, comprising mostly rolling scars, a reliable quality microneedling session, essential infusions and good doctor’s skill and experience can deliver 50 to 70% overall improvement on the average after 6 sessions. And you will notice substantial improvement as early as the second session.

severe scarring

For severe acne scars, this is a different story, microneedling can help start up new collagen formation, but definitely needs support from other treatments, combining subcision together with microneedling in a single session is a good option. This lessens the overall downtime. Alternating microneedling and fractional laser is also being done. You will need at least 4 sessions to see significant improvement.


So what made the difference? These are some factors that we noticed giving favorable results.

Combination of different acne scar treatment modalities always give better results.

Age, the younger the better. Skin Repair is better too. Old , wrinkled skin needs way more collagen to correct wrinkles, then the acne scars.

Newer acne scars improve faster than the older ones.

Smooth contoured improves the fastest , then the box scar (if combined with subcision) , lastly the ice pick scars.

Genetics really play a role, there are people who have good healing process and collagen formation.

Intake of Vitamin C and E, helps in the repair process.

Chubby faces respond better. wider and better plane and surface area for needle penetration.

Scars on the cheeks improve first .

Non smoking patients get faster healing time.

Microneedling  is a good starter treatment, it only has 3 days downtime, practically the shortest amongst the other acne scar treatments, minimally invasive, well tolerated and improves the 3 common types of scars, gives 50 to 70% improvement on mild to moderate scarring, long intevals between treatments, and now made more affordable.

A good discussion with your dermatologist is essential in knowing realistic expectations.

Hello.
Is microneedling suitable for little raised white scar on the chin 10 years old. I am scared that this can make my scar even more raised. Because after radio frequency treatment done on my face my scar became more raised.

What will happen if I derma roll over a port wine birthmark? If it's safe, what size roller
should I use? Can you please give some advice?
